Description

Provides a consolidated view to Health and Care Professionals of a Patient/Service User's complete and up-to-date records, sourced from various health and care settings.

Given that a Patient/Service User may have data recorded in many different systems, a Unified Care Record provides a means of bringing this data together, in one place, to enable Health and Care Professionals to have a holistic view of a Patient/Service User's health and care record. A Unified Care Record is intended to encourage a Patient/Service User-centred approach to care, and improve efficiency and safety as well as providing Health and Care Professionals with a comprehensive record that enables them to make more informed assessments and thus better care and treatment decisions.

This unified record can be made available to Health and Care Professionals involved in the direct care of a Patient/Service User and where there are data sharing agreements in place between the relevant data-sharing organisations. Data is drawn from the relevant systems and presented to the Health and Care Professional. The Unified Care Record may additionally provide the ability for the Health and Care Professional to tailor the view of the information to better meet their needs. This may include Patients/Service Users themselves or a Proxy acting on their behalf.


Outcomes

Patients/Service Users:
  • Will not have to repeat their previously reported medical history when they are seen by Health or Care Professional 
  • Will receive improved care because the Health and Care Professionals treating them are fully informed regarding their medical history
Health and Care Professionals involved in the care or treatment of the Patient/Service User:
  • Can view a complete and up to date Unified Care Record for a Patient/Service User bringing data together from different systems from a variety of health and care settings into a single record enabling them to assess and treat the Patient/Service User more safely and effectively
  • Can enable more integrated care across organisational boundaries through access to a common and shared view of the Patient/Service User's data

Given data for a Patient/Service User exists in systems across multiple organisations

When a Health or Care Professional accesses the Patient/Service User’s Unified Care Record

Then the Patient/Service User's data is retrieved from the systems

And the Patient/Service User data is displayed to the user

Acceptance criterion 2: understand source of Unified Care Record data

Given Patient/Service User data from multiple systems can be accessed via the Unified Care Record

When a Health or Care Professional accesses the Patient/Service User’s Unified Care Record

Then the Patient/Service User data is displayed to the user

And the source of the data can be identified by the user

So that I can be assured that everyone involved with my care has the correct and complete information about me

And the Patient/Service User has been given a means to access their own Unified Care Record

When the Patient/Service User accesses their Unified Care Record

Then the Patient/Service User’s data is retrieved from the systems

And their data is displayed

As a Health or Care Professional 

I want to be able to configure a default view when I access a Patient/Service User's Unified Care Record

So that I can immediately access the right information to support the health or care-related task I am undertaking

Acceptance criterion 1: configure default Unified Care Record view

Given the Unified Care Record solution can access Patient/Service Users’ data from multiple sources

When the display configuration is accessed

Then a default view can be set up by the user

and when that user accesses a Patient/ Service User's record they are presented with their default view
