Description

Enables Patients to share health monitoring data with Health or Care Professionals, to support the remote delivery of care, and increase self-care outside of clinical settings.

Outcomes

For a Patient:

Using medical devices and monitoring equipment at home or on the move to regularly record and share vital health information with Health or Care Professionals, and to receive support and assistance.

For Primary Care:

Remote monitoring of Patients to assist in diagnosis, support and decision making for prevention and anticipatory care.

For the wider NHS and social care system:

Supporting and facilitating the provision of health and social care in non-clinical settings.
