Supports Health and Care Professionals by providing tools to calculate risk levels for Patients, informing interventions to achieve better Patient outcomes.

Risk Stratification tools are specifically designed to identify those individuals who are at high risk of experiencing a future adverse event, such as readmission to hospital or unplanned hospital admission, or an acute event such as a heart attack.  Risk Stratification often takes into account multiple risk factors and morbidities and their inter dependency, rather than just concentrating on one criterion (e.g. high blood pressure).

A Risk Stratification Capability will allow risk calculations to be performed for individual patients using algorithms that can be run against Patient data (known algorithms can be used such as QRISK®2, QDiabetes®, QFracture®, QCancer®, QAdmissions®) to proactively identify a Patient's risk level (e.g. of an unplanned admission). If the high-risk individuals can be identified early and offered better support and preventive care then it may be possible to improve their health outcomes and their experiences of care while, at the same time, making net savings for the health service as a whole by averting complications and avoiding hospital admissions.

Outcomes

...

Supports the performing of Risk Stratification using pre-defined criteria that can be run against Patient data.

Outcomes

For a Patient:

Patients identified as at risk may receive preventative care.

For Primary Care:

Able to proactively identify at-risk Patients

...

  • Patients identified as at risk may receive preventative care

titleMUST Epics - Epics and acceptance criteria will be evaluated during the Capability Assessment Stage of Onboarding

C35E1- run Risk Stratification algorithms

As a Service Provider

I want to identify whether a Patient is at risk

...

.

MUST Epics - Describes the minimum functionality required to deliver a Capability. Solutions MUST be successfully evaluated against each Epic and Acceptance Criteria via Capability Assessment in order to be associated with this Capability

E00505 - perform Risk Stratification

As a Health or Care Professional

I want to perform Risk Stratification to identify Patients who are at risk

So that the management of their care can be prioritised

Acceptance criterion 1:

...

Given the Service Provider is using a Risk Stratification Tool

When they want to identify whether a Patient is at high risk of developing certain outcomes or diseases

Then they can run Risk Stratification algorithms against that Patient's data

...

perform Risk Stratification

Given the Health or Care Professional is permitted to perform Risk Stratification

When the Health or Care Professional selects to perform Risk Stratification

Then the risk level is calculated

And the results

...

Acceptance criterion 2: tools based on nationally or industry recognised algorithms

Given the Service Provider is using a Risk Stratification Tool

When there is a nationally or industry recognised algorithm for the Risk Stratification Tool

Then the Risk Stratification Tool uses the nationally or industry recognised algorithm

Acceptance criterion 3: view Risk Stratification Tool algorithm details

Given the Service Provider is using a Risk Stratification Tool

When the Service Provider chooses to view details about the algorithm used by the tool

Then the algorithm details can be viewed

And the source is displayed

E00506 - view criteria used for Risk Stratification

As a Health or Care Professional

I want to view the criteria used for Risk Stratification

So that I can review the criteria used to calculate the risk level for Patients

Acceptance criterion 1: view criteria used for Risk Stratification

Given the Health or Care Professional is permitted to perform Risk Stratification

When the Health or Care Professional selects to view the criteria used for Risk Stratification

Then the criteria are displayed

MAY Epics - Describes additional functionality associated with the Capability. Suppliers should consider all MAY Epics as part of their User Research. Suppliers can choose to map their Solutions to these Epics and they will be evaluated via Capability Assessment. Framework Authorities or purchasing organisations may require these Epics as product qualification or requirements criteria

E00507 - perform Risk Stratification based on national or industry recognised criteria

As a Health or Care Professional

I want to perform Risk Stratification based on national or industry recognised criteria to identify Patients who are at risk

So that I can prioritise the management of Patient care

Acceptance criterion 1: perform Risk Stratification based on national or industry recognised criteria

Given the Health or Care Professional is permitted to perform Risk Stratification

When the Health or Care Professional selects to perform Risk Stratification using national or industry recognised criteria

Then the risk level is calculated

And the results are displayed
